Duplicate W-2 Request Form    For State of Iowa Employees Only

Duplicate W-2 Wage and Tax Statement
To request a Duplicate W-2 Wage and Tax Statement, print and deliver a completed Duplicate W-2 Request form with a fee of $5.00 for each tax year requested, to DAS-Centralized Payroll. Types of payment accepted are cash or check payable to Treasurer, State of Iowa.
Please Note: Duplicate W-2 forms emailed to Centralized Payroll are not accepted due to confidential information required to process the request.
